| Zusammenfassung | Distortion product otoacoustic emissions (DPOAE) provide an objective and non-invasive examination of the cochlear function. However, the reliability of DPOAE tests at high frequencies has so far not been satisfactory due to stimulus calibration errors that may lead to invalid test results. While the commonly used in-the-ear calibration provides an efficient way to adjust the stimulus levels for each test ear by means of the DPOAE probe, it is prone to considerable errors above about 3 kHz. Accounting for the distance between the probe and the eardrum has long been a troublesome issue. The main objectives of this thesis are: to quantify the errors related to the in-the-ear stimulus calibration and characterisation of DPOAE probes; to search for ways to compensate for the impact of these errors on the DPOAE results; and to analyse the retrograde path taken by DPOAE signals. For this purpose, finite element models of both the DPOAE probe and the ear canal were created, sound pressure measurements on physical ear canal models were performed, and DPOAE measurements on human test subjects were carried out. Standing waves led to significant sound pressure variations along the ear canal. This effect was counterbalanced by referring the stimulus calibration to the eardrum position. Sufficiently accurate predictions of the sound pressure were achieved up to 8 kHz by employing the one-dimensional transmission line theory and using the acoustic length of the ear canal models for the transformation. The cross-sectional discontinuity at the interface between the probe and the ear canal models comprised another source of error, inducing a shift in the quarter-wave notch frequency. The impact of this discontinuity was interpreted as an "acoustic elongation" of the ear canal, which was substantial mainly for models of larger diameters. To compensate for the discontinuity effect, diverse modifications of the large-diameter eartips were suggested and implemented. Regarding the source characterisation of DPOAE probes, the impact of the number and lengths of the calibration cavities on the estimation of the acoustic input admittance of ear canal models was investigated. Concerning the choice of a calibration cavity set, a criterion was deduced that led to the estimation of the acoustic input admittance of an unknown test cavity with an accuracy of 0.3 dB. In a pilot study, a combined stimulation of DPOAEs via air and bone conduction was introduced. This stimulation resulted in detectable DPOAEs, and allowed - through swapping the acoustic signal paths - the identification of stimulus calibration errors. Moreover, the reference hearing threshold levels associated with a commercial DPOAE probe were determined, providing data for their standardisation. Further, the influence of the eartip impedance on the retrograde DPOAE path was analysed by impedance-tube measurements on specially designed foam specimens. Finally, the impact of varying the probe insertion depth within the ear canal was assessed and utilised to distinguish between physiological and instrumental distortions. This thesis sheds light on the currently unsatisfactory calibration of DPOAE probes, and provides a thorough assessment of the errors related to the source characterisation and the in-the-ear calibration procedures. It addresses both theoretical and practical procedures to reduce these calibration errors, thus paving the way for more reliable DPOAE results. Furthermore, it opens up alternative approaches to DPOAE stimulation that may prove to be clinically beneficial. Distorsionsprodukt-otoakustische Emissionen (DPOAE) erlauben eine objektive und nichtinvasive Untersuchung der cochleären Funktion. Allerdings ist die Zuverlässigkeit von DPOAE-Hörtests bei hohen Frequenzen bislang nicht zufriedenstellend, weil die Stimulus- Kalibrierung fehleranfällig ist und zu falschen Testergebnissen führen kann. Obwohl bei der Im-Ohr-Kalibrierung der Stimuluspegel für jedes Prüfohr mit der DPOAE-Sonde eingestellt wird, treten ab ca. 3 kHz erhebliche Kalibrierfehler auf. Die Entfernung zwischen der Sonde und dem Trommelfell stellt dabei den Kern des Problems dar. Die wesentlichen Ziele dieser Arbeit bestehen darin, die bei der Im-Ohr-Kalibrierung und Charakterisierung von DPOAE-Sonden auftretenden Fehler zu quantifizieren; nach Wegen zu suchen, um den Einfluss dieser Fehler auf die DPOAE-Ergebnisse weitgehend zu kompensieren; und anschließend den retrograden DPOAE-Signalweg zu analysieren. Für diesen Zweck wurden Finite-Elemente-Modelle der DPOAE-Sonde und Gehörgänge entworfen, Schalldrücke an physikalischen Gehörgangsmodellen aufgenommen, und DPOAE-Messungen an Versuchspersonen durchgeführt. Stehende Wellen führten zu erheblichen Variationen des Schalldrucks im Gehörgang. Dieser Effekt wurde kompensiert, indem die Stimuluskalibrierung auf die Trommelfellposition bezogen wurde. Vorhersagen des Schalldrucks mit ausreichender Genauigkeit konnten bis 8 kHz mit der eindimensionalen Wellenleitertheorie und unter Verwendung der akustischen Länge der Gehörgangsmodelle bei der Transformation erzielt werden. Der Querschnittssprung zwischen Sonde und Gehörgangsmodellen bewirkte eine Verschiebung der Viertelwellenlängen-Frequenz, was als "akustische Verlängerung" der Gehörgangsmodelle interpretiert wurde, die vor allem bei Modellen größeren Durchmessers auftrat. Um diese Verschiebung zu kompensieren, wurden verschiedene Modifikationen der Ohreinsätze größeren Durchmessers vorgeschlagen und realisiert. Bei der Quellcharakterisierung von DPOAE-Sonden wurde der Einfluss der Anzahl und der Länge der Kalibriervolumina auf die Abschätzung der akustischen Eingangs- admittanz der Gehörgangsmodelle untersucht. Es wurde ein Auswahlkriterium für die Kalibriervolumina angegeben, welches die Abschätzung der Eingangsdmittanz eines unbekannten Testvolumens mit einer Genauigkeit von 0,3 dB erlaubte. In einer Pilotstudie wurde eine kombinierte Stimulation von DPOAE über Luft- und Knochenleitung vorgestellt. Diese Stimulation erzeugte messbare DPOAEs, und erlaubte - durch Vertauschen der Signalübertragungswege - eine Identifikation von Fehlern bei der Stimuluskalibrierung. Zudem wurden die äquivalenten Bezugshörschwellen für eine handelsübliche DPOAE-Sonde bestimmt und somit Daten für ihre Standardisierung bereitgestellt. Ferner wurde der Einfluss der Impedanz des Ohreinsatzes auf den retrograden DPOAE-Weg mit Impedanzrohr-Messungen an speziell entworfenen Schaumstoffproben analysiert. Anschließend wurde die gezielte Variation der Einstecktiefe der Sonde im Gehörgang untersucht. Deren Ergebnisse wurden verwendet, um zwischen physiologischen und technischen Distorsionen zu unterscheiden. Diese Arbeit analysiert die derzeitig nicht zufriedenstellende Kalibrierung von DPOAE-Sonden und stellt eine ausführliche Untersuchung der bei der Charakterisierung und Im- Ohr-Kalibrierung auftretenden Fehler dar. Sowohl theoretische als auch praktische Verfahren, um diese Fehler zu reduzieren und somit die Zuverlässigkeit der DPOAE-Ergebnisse zu erhöhen, wurden behandelt. Ferner wurde mit der kombinierten Stimulation von DPOAEs ein neuer Weg eröffnet, der Vorteile für den klinischen Einsatz mit sich bringen kann. Las otoemisiones acústicas por productos de distorsión (DPOAE) constituyen un método objetivo y no invasivo para evaluar la función coclear. Sin embargo, la fiabilidad de las pruebas DPOAE a altas frecuencias no ha sido satisfactoria hasta ahora, debido a errores de calibración de estímulo, lo que puede causar una prueba no válida. Mientras que la calibración intra-auricular común exige una forma eficiente para ajustar los niveles de estímulo para cada oreja de prueba por la sonda DPOAE, es propenso a errores considerables por encima de 3 kHz. La contabilización de la distancia entre la sonda y el tímpano ha sido un tema problemático. Los principales objetivos de esta tesis son: cuantificar los errores en la calibración intra-auricular y en la caracterización de sondas DPOAE; buscar formas para compensar estos errores y reducir su impacto en los resultados de DPOAE; y luego analizar las señales DPOAE retrógradas. Para ello, los modelos de elementos finitos tanto de la sonda DPOAE como del conducto auditivo fueron creados, las mediciones de presión sonora realizadas en modelos físicos del canal auditivo y las mediciones de DPOAE en sujetos de prueba humanos. Las ondas estacionarias han dado lugar a variaciones significantes en la presión sonora a lo largo de los modelos de conducto auditivo. Este efecto fue contrarrestado por referencia de la calibración de estímulo a la posición del tímpano. Predicciones suficientemente exactas de la presión sonora se han alcanzado hasta 8 kHz mediante la teoría de la línea de transmisión y el uso de la longitud acústica para la transformación. La discontinuidad de sección transversal, en la interfaz entre la sonda y los modelos del conducto auditivo, formó otra fuente de error, induciendo un cambio en la frecuencia de cuarto de onda. El impacto de esta discontinuidad fue interpretado como una "prolongación acústica" del conducto auditivo, lo cual fue importante sobre todo para los modelos de mayor diámetro. Por lo tanto, se propusieron y se llevaron a cabo diversas modificaciones de las olivas de gran diámetro para compensar esta discontinuidad. Con respecto a la caracterización de fuente de las sondas DPOAE, se investigó el impacto del número y de la longitud de las cavidades de calibración sobre la estimación de la admitancia acústica de entrada de los modelos del canal auditivo. En cuanto a la elección de un conjunto de cavidades por calibración, se dedujo un criterio que permitó estimar la admitancia acústica de entrada con una precisión de 0,3 dB. En un estudio piloto, se presentó una estimulación combinada de DPOAE por vía aérea y vía ósea. Esta estimulación resultó en DPOAE medibles, y permitó - al intercambiar las vías de las señales acústicas - la identificación de errores de calibración de estímulo. Por otra parte, se determinaron los umbrales de audición de referencia por una sonda DPOAE comercial, representando datos para su estandarización. Además, la influencia de la impedancia de la oliva sobre el camino DPOAE retrógrado se analizó por medidas con tubo de impedancia en muestras de olivas de espuma de diseño especial. Por último, el impacto de la variación de la profundidad de inserción de la sonda en el conducto auditivo fue evaluado y luego utilizado para distinguir entre las distorsiones fisiológicas e instrumentales. Esta tesis da luz sobre la calibración actual insatisfactoria de las sondas DPOAE, y proporciona una evaluación exhaustiva de los errores relacionados con los procedimientos para la caracterización de fuente y para la calibración intra-auricular. Se analizan los aspectos teóricos y prácticos para reducir estos errores, allanando así el camino para obtener resultados de DPOAE más confiables. Además, abre caminos alternativos para la estimulación de DPOAE, lo que puede llegar a ser clínicamente beneficioso. |
