Zusammenfassung To study the glutamate hypothesis of schizophrenia we performed proton MR spectroscopy at 3 tesla in hippocampus and different cingulate regions of schizophrenic patients and healthy controls, focusing on quantitation of both glutamate and NAA. Eighteen schizophrenic patients were compared with 30 healthy age matched volunteers. 1H-MR spectra were acquired using PRESS optimized for glutamate detection from 3 voxels including the left hippocampus, the anterior and the posterior cingulate gyrus. For metabolite quantification a procedure developed inhouse was employed. The glutamate concentration in the hippocampus was significantly higher (p = 0.001) and the NAA concentration significantly lower (p = 0.016) in patients than in controls whereas there were no significant differences for the other voxels. These results confirm recently reported glutamate alterations in hippocampus in schizophrenia. Being accompanied by a reduced NAA level in this brain voxel, this finding suggests dysfunctional glutamatergic neurotransmission with concomitant disturbed neuronal integrity in a region playing a key role in the pathophysiology of schizophrenia.
